Expert faculty will present a topic of relevance and importance to primary care physicians.  Live virtual activities are currently presented on Zoom via CloudCME. The general format of the one hour activity is 30 minutes of didactic lecture, 15 minutes of case presentation, and 15 minutes of Q & A with the faculty. This activity is a recorded version of the live activity and it doesn't allow a live Q&A.  You will be asked to attest to your attendance and expected to complete an evaluation form in order to receive CME credit for your attendance.

NOTE: Learners may not claim CME credit for an Enduring Material if they have claimed CME credit during its live program.

1. Define cancer survivorship and describe its scope within the continuum of cancer care.
2. Develop strategies to monitor, prevent, and manage the physical, psychosocial, and immunologic effects of cancer and its treatment, including fatigue, cognitive dysfunction, sexual dysfunction, pain, and lymphedema.
3. Apply evidence-based approaches to surveillance for recurrence, metastasis, and subsequent primary cancers (tailored to patient risk factors and cancer type).
4. Formulate comprehensive survivorship care plans that summarize treatment history, outline follow-up recommendations, and clarify delineation of care coordination among oncologists, PCPs, and subspecialists.
